Intake & Check-up forms

2 min read

1. What is the difference between Intake and Check-up forms?

Form type

Purpose

Frequency

Intake

Used for the first introduction or initial assessment of the member.

Shared once at the start of the coaching process.

Check-up

Used to receive periodic feedback about progress, experience or other relevant topics.

Can be scheduled periodically (e.g. weekly or monthly).

2. Where can I create and configure these forms?

  • Creating forms: Create templates for both Intake and Check-up forms through Settings → Intake and check-up forms.

  • You can create multiple forms to assign different forms to different types of members. For example, you may want a different check-up form for a member who wants to lose some weight alongside a busy work and personal life than for a professional athlete.

  • You can add multiple sections to cover different topics within your form. This keeps it clear and organized.

  • The forms are linked directly to the 'Goals' and 'Measurements' pages in the member profile. Members can use the intake form to add their starting weight directly to their profile, or use the check-up form to enter weekly measurements such as weight, resting heart rate or HRV.

  • Assigning forms: Assign a form to a specific member through the buttons Intake or Check-up in the member profile.

3. How do I set up an Intake form?

When assigning the form in the member profile under 'Intake':

  • Select the desired form through 'Assign now'.

The form is now visible in your member's profile. If it has not been completed after some time, you can send a reminder here.

3. How do I schedule a recurring Check-up?

When assigning the form in the member profile under 'Check-up':

  • Select the desired form through 'Check-up form'.

  • Set the frequency through 'Frequency' (for example, every Monday).

You can set multiple reminders. Remember to save your changes afterwards and click 'Add now' to finalize adding the check-up form, including reminders. Then check whether the 'Check-up reminder' toggle is on. If so, the check-up is visible in the member app's 'Progress' section.
