Creating a check-up form

The check-up form completes the coaching loop. Send it to your member periodically (e.g. weekly) to ask about their experience and progress.

📍 Go directly to forms: coach.fitsociety.io/company/settings/forms

Step 1: Create a form

  1. Go to Settings (⚙) → Intake and check-up forms.

  2. Click ✦ Generate form with AI.

  3. Describe what you want to know, e.g.: "Short form to ask the member about their experience each week."

  4. Set the number of topics to 1 and questions to 5. Keep a check-up short so members actually complete it.

  5. Click Generate.

  6. Click Save changes in the top right when you are satisfied.

Want to create a form manually? Click + Create form.

  1. Open the check-up form and click the tab Goals.

  2. Click + Add goals.

  3. Under Body information select the field Weight (and others if desired).

  4. Click Save.

Weight is then automatically requested at each check-up and appears as a graph in the member's Progress tab.

Step 3: Set up the check-up for each member

  1. Go to the member profile → tab Check-up.

  2. Click Add form and select the check-up form.

  3. Click Add now.

  4. Under Recurrence set: Custom → every 1 week, and choose a fixed day (e.g. Sunday).

Step 4: Add a reminder

  1. Click Add reminder.

  2. Choose a channel: email or chat.

  3. Set the time and customize the text, e.g.: "Your weekly check-up is ready! Go to Progress in the app."

Please note: the reminder does not contain a direct link. Members find the check-up through Progress → Check-up available in the app.

Central check-up overview

You do not need to check each member individually for completed check-ups. FITsociety has a central overview of all submissions.

📍 Go directly to check-ups: coach.fitsociety.io/member/checkup

Where can you find it?

In the left menu, click Coach → Check-ups.

What do you see?

A list of all recent check-up submissions from all your members, sorted by date. Each check-up shows the member's name, submission date and whether feedback has already been given.

Set aside a fixed time each week (e.g. Monday morning) to review the overview. Open a check-up, read the answers, compare them with the previous week and send feedback through the chat. This lets you process all weekly check-ins quickly.

Difference between check-up and intake

  • Check-up → periodic (weekly) → member finds it through Progress → Check-up available in the app. Coach sees all submissions through Coach → Check-ups.

  • Intake → once → member accesses it through Profile → Intake in the app. Coach sees submitted intakes through Coach → Intakes.
